Secondary screen completely black

I'm going to guess there's a different setting in its eqclient.ini file. I would go into the EQ1 folder and find the eqclient-<CharacterName>.ini files, and copy a good one to replace the probably bad one, and see if that helps.

Another diagnostic that could provide useful info is in the In-game ISBoxer Control Panel (Ctrl+Shift+Alt+G), on the debug tab, click "Capture State". You can Paste the results back here for me.

You said you tried to use the fully installed standalone game client (e.g. not Steam). Could we see a capture state when using that game client to run your characters?

I had this issue as well, except it was with Diablo 3. I reverted back to an older Nvidia driver and that fixed this issue for me.

I would love to roll back my driver, but I'm unsure of which one to roll back to. I took a break from the game for a few months, and it worked then. Just need to find out when I stopped and it was still working.

Find a driver around the beginning of December or end of November. Those will work. For some reason, after that, we get this problem. Don't forget to do a "clean" install.

Had to revert back to a driver from early December... Now I have to figure out what was patched in so I can try and fix it.

I'm hoping it will be fixed by the next driver update with a security fix. Usually, the ones in between security fixes are just compatibility fixes for certain games so I don't worry too much about those.
