I always use the wizard and set it up. I will check out what it says, I never mess with that portion.

Under window layouts it says 3440x1152 which my desktop is 3440x1440
For troubleshooting, I changed the in game resolution to my desktop 3440x1400 and it is doing the same thing. my DPI is set to 100%. This is driving me bonkers. This should work just fine.

Your main region should be the same as the resolution in game.
So beyond what MiRai says, from your brief description, it would appear the resolution your games should be at is 3440x1152. Presumably you have smaller windows underneath the main region, hence the difference in resolution between 3440x1440 and 3440x1152.
However, Neverwinter is an older game, and it may not like working at such an oddball ratio (215 : 72), and may prefer something a bit more "normal. I don't really know, but may be worth checking, after testing out the windowed modes.
