Okay. The answer does not lie in your ISBoxer profile itself, but rather some sequence of events happening in the game. So I'm going to have to do some guessing and throw some random things here about things you might be doing:
My first thought is that for this to happen from your Alt+D key, Mouse2 (Right click, for mouse look) would probably need to be pressed. That is, unless you have a custom strafe key that might also be on D, especially Alt+D. If you have Repeater enabled, or have the mouse over a Repeater Region, any modifiers you're holding are also passed to the Repeater Target (so that if you shift+click, that window gets a shift+click). So if your mouse is over a Repeater Region that goes just to your healer, and you hold Alt+D, then your healer is going to get the Alt that you are holding, independently from the D->all other. (Note that for Mapped Keys that DO NOT have the Hold option enabled, that "D->all other" would override the Alt you are broadcasting; this is specifically because the Hold option is enabled when sending D, it cannot drop the Alt)
