mouse off a little plz help

heres a pic explains it all and i dont know how to fix
acc 1 and acc 2 are good
its acc 3 and acc4 that are off

Try toggling the in-game window settings from fullscreen to windowed and back, or vice-versa.

While you're toggling between display modes, Diablo III pretty much requires Windowed (Fullscreen):
