The same way you set up other FTL keys. However, although it is convenient to use a mouse button as a Hotkey, you probably do NOT want it passed as the Key Combination. That's because the click may end up hitting a UI element depending on where you left the cursor in that window.
To set this one up, make a copy of one of the existing FTL DPS Keys (e.g. "DPS 1"). Set the Hotkey to the extra mouse button (Mouse1, Mouse2, Mouse3, Mouse4, Mouse5 are supported, any more than that and it's a special button on the mouse that would probably need to be configured as a keyboard button), you can just click in the auto-detect box. Then in Step 1 instead of "1 -> all" send something else (if you want). Just don't send the mouse button. You can make it send the same thing as one of the existing mappings as you want.
