Everquest Key map doesnt work on 1 key

I six box and key map for my 1 key is assist>pet attack. The first 5 instances work fine but the sixth is a crap shoot, sometimes working perfectly but more than not, requiring multiple presses to activate. I've done all that I can to make sure everything is exactly the same on all instances, but I cannot find a solution. Anyone have this before?
