Make sure the LOTRO launcher is not still running (there should be an option to make it close after launching the game), and make sure the game is in Direct3D9 mode. You may also need to use the development (test) build of Inner Space.
That's really about it. The second one will launch when IS has detected the first one is launched. If it took longer than 60 seconds to get the first one in game, manually launch the next character by re-selecting the Character Set from the menu.

Ahh interesting, so it's crashing for you. This is on Windows XP? I take it from the debug box that you have a debugger installed, do you know how to attach the debugger and retrieve the call stack? If so, that would help. I don't think it crashes there for anyone else, so I'm assuming it has something to do with software that's installed on your PC...
