Patching bug?

Without Innerspace or ISBoxer running I started ISBoxer, then right clicked the character set and selected launch.

Innerspace launched my first LoTRO client, then started patching and reported an error Innerspace.dll could not be patched (I think)

It seems like Innerspace shouldn't have started running the launch script before it had decided to patch or not.

Yeah, that is true. It's going to lose the instruction to launch the team entirely if it patches, though. On a normal basis, you will either have Inner Space running already or it's typically stable for a period of months at a time. On that note, if you have development builds selected, you can return to using the live build; they are currently the same build.
