Also misalligned for me. Would love to run my main window on max res, but as of right now having to go with pretty crappy res and game looks slightly better than D2 lol

In the top left pane: expand repeater profiles, left click default. In the bottom right pane: open the drop down menu labeled "cursor positioning mode" and select "scaled to fit".
